Researchers at Universiti Malaysia Kelantan have identified a significant medical application for a creature many consider merely a nuisance: the buffalo leech. A comprehensive three-year investigation has demonstrated that proteins extracted from the saliva of the local buffalo leech species, Hirudinaria manillensis, possess anticoagulant properties comparable to pharmaceutical blood thinners used worldwide. The discovery could revitalise Malaysia's struggling leech farming sector and position the country as a supplier of naturally-derived anticoagulant compounds to global pharmaceutical markets.

Dr Zulhisyam Abdul Kari, a senior lecturer at the Faculty of Agro-Based Industry at UMK, explained that the key compound identified in the leech saliva is hirudin, a naturally occurring substance that functions similarly to heparin, the synthetic anticoagulant widely employed in modern medicine to prevent blood clots. Laboratory analysis confirmed that protein extracted from buffalo leech saliva effectively delays blood coagulation, performing at levels comparable to existing anticoagulant treatments. This finding validates decades of traditional medical practices while opening pathways for industrial-scale pharmaceutical development.

The research team's decision to focus on buffalo leeches rather than terrestrial varieties reflects a strategic understanding of local biology. Buffalo leeches produce significantly larger volumes of saliva with markedly higher protein quality and concentration than land-dwelling species, making them a more efficient source material for extracting bioactive compounds. This biological advantage positions Malaysian buffalo leeches as a potentially superior raw material compared with leech populations found elsewhere in the region.

Historically, leech-based therapies have addressed diverse health conditions across traditional and modern medical systems. Beyond anticoagulation, medical practitioners have employed leeches to treat gout, acne, and chronic wounds in diabetic patients, with therapeutic benefits documented across numerous case studies. The quality of leeches sourced from Asia has earned recognition among international medical institutions, with developed nations including Germany relying heavily on Asian leeches as preparatory treatment prior to cardiac surgery, ensuring optimal blood circulation during complex procedures. Thailand represents another significant regional market, regularly importing Malaysian leeches to supply its domestic medical sector. This established international demand demonstrates the commercial viability of Malaysian leech products and the potential for expanding export markets.

Despite these opportunities, Malaysia's leech farming industry has contracted substantially over the past decade, reversing earlier expansion driven by government-supported contract farming schemes. The deterioration stems partly from fraud and misconduct that eroded investor confidence and destabilised the sector's reputation. These trust deficits have deterred domestic capital investment and stalled the development of formal farming infrastructure necessary for scaling production to meet international demand.

Environmental pressures compound the industry's challenges. Wild leech populations, particularly in traditional habitats such as paddy fields, have declined markedly as intensive agricultural practices intensified. The widespread deployment of pesticides and synthetic chemical fertilisers has devastated natural leech ecosystems, threatening species survival in their native environments. This ecological degradation reduces the potential for wild-harvesting as a supplementary production strategy and underscores the necessity of establishing managed farming operations.

Regulatory authority over leeches in Malaysia shifted in 2011 when responsibility transferred from the Department of Wildlife and National Parks to the Department of Fisheries Malaysia, reflecting recognition of leeches' commercial and resource value. This institutional reorganisation created opportunities for more industry-focused management, yet the transition alone has not reversed the sector's decline. Dr Zulhisyam emphasised that substantially increased research funding remains essential to advance molecular biology investigations and unlock the full commercial potential of local leech species for the global biotechnology and pharmaceutical sectors.

The researcher's advocacy for leech conservation reflects a broader philosophical reorientation necessary within Malaysian society. Leeches, typically viewed with revulsion or indifference, represent a valuable natural resource with significant scientific and commercial applications. Recognising their ecological and pharmaceutical value requires educational outreach and policy frameworks that incentivise preservation rather than eradication. Every organism, Dr Zulhisyam argued, occupies an irreplaceable niche within ecosystems and possesses inherent utility for human applications yet undiscovered.

For Malaysia, the implications extend beyond a single agricultural commodity. The country's biodiversity, particularly in tropical regions, harbours numerous species with unexplored pharmaceutical properties. Developing leech farming into a knowledge-intensive biotech industry could establish templates for commercialising other natural compounds and building competitive advantage in global pharmaceutical markets. Investment in molecular biology research on local leech species would generate intellectual property, create high-value employment, and demonstrate the economic returns available through systematic bioprospecting of native fauna.

The convergence of traditional knowledge with contemporary scientific validation offers a pathway for Malaysian researchers to contribute meaningfully to global pharmaceutical innovation while simultaneously revitalising rural livelihoods. Success would require coordinated support encompassing regulatory clarity, research funding, institutional capacity-building, and investor confidence restoration.
